Documents obtained by NBC 5 claim Kimberly Smith "placed a rag in the mouth of Jessica Smith in an attempt to smother her and then threatened to kill Jessica and herself with a knife" on February 6, 2012.

According to a witness quoted in the affidavit, Kimberly Smith locked herself and Jessica in a walk-in closet in an upstairs bedroom after attempting to smother Jessica with a rag.

The witness confronted Kimberly Smith and forced her to release Jessica from her grip before displaying a knife and telling Jessica and the witness that "she (Kimberly) was going to kill Jessica and then herself."

The witness, whose name is redacted in the released documents, said they did not report the incident to police "because they felt the incident was over."

That same witness returned to the house on Feb. 7 to find Kimberly and Jessica were gone. Shortly after confirming Jessica was not at her school, an Amber Alert was issued for the 11-year-old.

Kimberly Smith faces charges of aggravated assault and child endangerment due to these events.
